New Opioid Laws and Helpful Forms

Effective July 1, 2018, Tennessee state law (SB 2674/HB 2348 and SB 2257/HB 1831) requires informed written consent be obtained from a patient prior to prescribing more than a 3-day supply of opioids to a patient, or prior to prescribing an opioid to a woman of childbearing age.

In addition, click here for a templated informed consent form from the American Academy of Pain Medicine as a resource. Physicians and other prescribers are advised to have this form reviewed by legal counsel prior to use and regularly reviewed thereafter.
