Register to Become a COVID-19 Pandemic Vaccinating Provider
Provider defined as those who come into contact with an environment where he/she is exposed to COVID-19 (housekeeping would be added, for example).
If you do not meet any of these criteria
We are still awaiting more information from city, county and Shelby County Health Department. You will likely be vaccinated at a public location. This could include hospitals, pharmacies, FQHC’s or other to be determined public locations.
