Director of Physical Therapy

Reports to: Medical Director and Practice Administrator

Purpose of Position and Scope of Responsibility: The Director of Physical Therapy is responsible for the assessment and treatment of the assigned caseload. Direct and supervise the scheduling and good flow of patients within the rehabilitation department. Under the direct supervision of the Medical Director, responsible for patient care in the rehabilitation setting.

Essential Competencies/Performance Expectations include, in addition to the technical skills outlined in the attached competencies, demonstrated application of company mission and core values, demonstrated competence in communication, professionalism, organizational and analytical skills, and knowledge in performance of duties and responsibilities to the patient population served, CARF coordinator for patient care, follow-up and survey, including development of individual patient programs, delegating program precertification, initiating IPRP follow-up and survey review. For development of individual programs, see the Physical Therapist’s competencies. For follow-up and survey review, patients will be contacted by phone or mail to complete appropriate survey at the time of initial D/C, one month, three months, six months and one year post D/C. Stakeholders and physicians will be contacted by mail or through the patient to complete appropriate surveys, conducts physical therapy comprehensive assessments to determine the patient’s level of function in areas including gross mobility, physical status, ambulation, range of motion upon admission and performing reassessments as necessary.
Establishes and carries out individualized treatment plans upon completion of comprehensive assessment phase and makes personnel assignments to adequately cover patient load. Writes daily progress notes including documenting patient’s treatment and response to treatment. Maintains records and prepares reports according to facility procedures and IPRP requirements. Participates in client care conferences, family conferences and IPRP team meetings.
Demonstrates professional interaction with patient, family members and staff. Supervises staff Physical Therapist and Physical Therapy Assistant and Technician in direct and indirect client care, including performance appraisals. Provides input and assists in implementation of program development on an ongoing basis, conducts staff conferences as needed to discuss professional or personnel problems to maintain a well-coordinated and integrated service among all treatment areas.

Qualifications: Experience/Education/Certification: Must have current, unencumbered practice licensure. Graduate from an accredited Physical Therapy program. CPR certified. Minimum of 3 years experience is required for this position.
